The State of Oregon has published an Advanced Directive form that allows you to identify a healthcare representative as well as identify your preferences relating to end of life choices. I have included a link below that connects you to the Oregon site. You can print it out here or you can get a copy from any hospital or healthcare professional in Oregon. It is a government form. You do not have to fill one out. The benefit of the form is that it allows you to designate someone to make healthcare decisions for you if you are not able to do so. If you decide to fill out the form and sign it, be careful to follow the specific rules set forth in the last two pages that limit who can witness your signature.
